Please contact the Pine Lake registrar or the Issaquah School District for information regarding enrollment requirements and to schedule an appointment to receive a registration packet.

National Junior Honor Society - Community Service Form

Sports Forms

Sport Participation Form - required every year.
       Parents complete Sections I, II, III
       Students and parents complete section IV
       Physician must complete Section V*.

*Physical exams are valid for 12 months from the date of the exam. Physical Update Form may be completed by a physician and extend the physical exam another 12 months.
